Age of the deceased at the time of death

Sex (male or female)

Marital Status

Usual place of residence of deceased

Date of death

Place of death

Registration number

Registration date

Date of issuance

Photocopy of a Registration of Death

 

Photocopies are rarely needed by citizens and are, by law, for restricted use only (Note: They are generally only required for genealogical, court or consulate purposes. They are not for use as identification.)

 

This is a copy of the registration completed at the time of death, by the spouse, next-of-kin or close relative

 

The photocopy print size is 21.5 X 28 cm (8 1/2 X 11 in.) (there is always an official raised seal and red stamp on the back stating it is a certified true copy of the original registration)

 

The information contained on the photocopy depends upon the year the event occurred as Vital Statistics has changed the form and the data it collects over the years

 

It will, however, always contain the following information: 

 

Full name of the deceased

Sex (male or female)

Date of death

Place of death

Marital status of the deceased at the time of death

Burial information

Usual place of residence of the deceased

Registration number

Registration date

Medical Certificate of Death

 

This is a photocopy of the Medical Certificate of Death completed at the time of death, usually by the attending physician or medical examiners office as is appropriate

 

The photocopy print size is 21.5 X 28 cm (8 1/2 X 11 in.) (there is always an official raised seal and red stamp on the back stating it is a certified true copy of the original registration)
